Home Care Client Manager

About the Position
We are looking for a Home Care Client Manager to join our team in Cleveland, OH. This is a full-time, on-site position for someone who is organized, compassionate, dependable, and comfortable taking ownership of client needs.
The Client Manager serves as an important connection between our clients, their families, caregivers, and office team. You will help ensure that clients receive the right care, that caregivers have the support they need, and that concerns are addressed quickly and professionally.

This is a hands-on position where no two days are exactly alike. You may spend part of your day communicating with families, reviewing a client's needs, coordinating caregivers, completing a home visit, updating a care plan, and helping resolve an unexpected scheduling or care issue.

Responsibilities:
  • Build strong, trusting relationships with clients and their families.
  • Complete client intakes, assessments, and ongoing care evaluations.
  • Develop and update individualized care plans based on each client's needs.
  • Communicate regularly with clients, families, caregivers, healthcare professionals, and other involved parties.
  • Conduct in-home visits to check on clients and ensure quality of care.
  • Monitor client needs and identify changes that may require adjustments to services.
  • Coordinate with the scheduling team to help ensure appropriate caregiver coverage.
  • Support, train, and communicate with caregivers regarding client needs and expectations.
  • Address client and family concerns promptly and professionally.
  • Maintain accurate client records, care documentation, and follow-up notes.
  • Identify gaps in care and work with the team to find practical solutions.
  • Help ensure services are provided according to company policies and applicable regulations.
  • Participate in quality improvement and help maintain Hummingbird's high standards of care.

Qualifications:
  • Previous experience in home care, healthcare, case management, care coordination, client management, or a related field.
  • Experience working with older adults or individuals who require assistance at home is strongly preferred.
  • Knowledge of basic caregiving practices and home care standards.
  • Strong organizational, communication, and time-management skills.
  • Ability to maintain confidentiality and handle sensitive client information professionally.
  • Proficiency with email, Microsoft Office/Google Workspace, scheduling software, and electronic documentation systems.
  • High school diploma or equivalent required.
  • Associate or bachelor's degree in nursing, social work, healthcare administration, or a related field is preferred.
  • Nursing, social services, healthcare administration, or supervisory experience is a plus.
